RCA Executive Overview Workshop: Round table Discussion - Trinidad

March 24, 2020 – 1 Day Workshop Executive Round Table Discussion
$450 pp/USD Location: House of Angostura, Trinidad

RCA Exec. Workshop

Workshop Abstract: The presentation will open with a discussion about the core steps of the PROACT Investigation Management System. The workshop leader will then delve into the criteria defining a ‘thorough and credible’ RCA. A contrast will be made between the pros and cons of the most common RCA graphical expressions (5-Whys, Fishbone Diagrams, Logic Trees). Attendees will participate in the analysis of a basic case study, which will then be applied to the most common RCA expressions. This will allow attendees to understand why some analytical tools are more comprehensive than others, and when each should be applied. This session will provide the attendee a quality control checklist to use, to evaluate RCA presentations that are given to them. The workshop will conclude with how to make an ROI business case for an RCA, so these expectations can be set for those analysts in the field.

Very complete course on how to perform RCA from start to finish. Real life examples of problems are excellent in making connections.

Sanya Mathura

“Mark, It was an absolute pleasure to see you deliver the BFA course yesterday.

The manner in which you made the topics come to life with your experiences and knowledge was great to behold. You continuously connected with the audience and had them engaged for the entire day! (Which is a bit tough to do especially after lunch). Your wealth of experience and knowledge throughout the day really helped in bringing the participants to the understanding of the PROACT methodology and the concepts that were covered in the BFA. “

The PROACT® Logic Tree, is one of the best methods we use. We are proud that all SABIC sites and corporates are unified in terms of analyzing the major incidents by using PROACT® Logic Tree methodology since our aim and target is always to reach to the latent root cause(s) that failed to protect our assets from any false human interaction.

“ I’ve used the PROACT method for RCA many times and it has always surfaced issues with leadership decisions and omissions related to guidance (policies, processes and procedures) and resources.”

Wonderful system! Presentation was done in a very consistent manner giving a good overview of system and capabilities. The real case scenario exercise was a good way to bring it all together.

“PROACT RCA – A technique that allowed us to fix a chronic processing problem that plagued one of our gas plants for more than ten years. If not for this technique, we would still be fire fighting this problem today.”

Sam McNair

“I rank your organization as the best in the world at what they do and very often advise my clients to train on your software and methods. In 33 years I have come across none better. But as good as the tool is, it is not the only tool needed to have a well-balanced toolbox.”

Sam McNair Director Of Operations at Lagniappe Professional Services LLC

Reginald Gullette

I definitely learned new ways / improved ways to think while troubleshooting on the job. It inspired me to break away from staying in the box mindset.

Annelise Zeemann

“All materials tell metallurgists what they suffered during a failure, and…as I am a good listener…it is easy for me to know why a material failed; but we know that the definition of a damaged mechanism is just part of the story…and without a good RCA the material answer is nothing.”

Annelise Zeemann, Materials Life
