What is the PROACT® v3.6 Suite?

LEAP™ Basic
FMEA and Opportunity Analysis:
This
software provides a way for you
to examine your data sources for the Significant
Few opportunities. These are the 20% of the
events that are costing 80% of the losses. Losses
are defined by the user and can range from loss
of reputation to the annual cost of chronic events.
The outcome is a chart that graphically depicts the Significant
Few and suggests that these events are the most qualified candidates
for the credible and thorough Root Cause Analysis.
LEAP™ facilitates the FMEA and Opportunity Analysis processes,
while all the time documenting the inputs. When all the inputs
have been satisfied the data is compiled by the program and the
LEAP™ report is complete and ready for printing.

PROACT® RCA
software is an automation tool that facilitates
analysts' through the RCA process no matter what
the undesirable outcome. PROACT® is an acronym that stands
for:
PReserving Event Data
Ordering the Analysis Team
Analyzing Event Data
Communicating Findings & Recommendations
Tracking For the Bottom-Line Results
Each element of PROACT® is essential
to any analysis process. You cannot determine the cause of
an undesirable event without information. This makes preservation
of data a key initial step in any analysis process.
Organizing a team is hard enough in itself with the lean staffs
we have these days, but organizing an ideal RCA is even more
difficult. Having the right members on a team can make the difference
between success and failure. Teams must know why they are together
and how they know when they have been successful.
Analyzing the data collected in preserve
is like the detective who has collected evidence and leads
and now needs to know how to "put it all together". PROACT® utilizes
an extremely easy to use drag-and-drop logic tree to represent
the sequence of errors that lead to failure. The intuitive
rules built into the program are based on prominent human error
researchers from around the world. These rules guide the analyst
through developing the actual logic sequence that led to failure.
Each hypothesis developed must be validated with factual data
and logged in our verification log.
Eventually the intuitive aspects of the
program will prompt the user to identify Physical Human and
Latent Roots. For example, we may find after an analysis that
an improper medication was given to a patient and it resulted
in a sentinel event. A Physical Root may be that the wrong
medication was administered. A Human Root may be that the nurse
improperly transcribed the label on the vile (error of commission).
A Latent Root may be that the label on the vile utilized all
capital letters and increased the risk of improper transcription
due to Human Factors deficiencies. The goal of RCA is to identify
deficiencies in systems not to "witch
hunt" individuals or groups.

Communicating findings and recommendations
is vital to any analysis. If we cannot effectively sell our
recommendations to management then the causes identified run
the risk of recurrence. PROACT® prompts
the user to fill in specific fields related to causes identified
and the recommendations to overcome them. The user has the option
to print sections of the report, or with one keystroke, print
the entire report and all the supporting data. PROACT® also
features an on-line presentation mode so that analysts can electronically
present their findings to a management group without the hassle
of developing a Power Point® presentation.
Tracking for results in monitoring the
effectiveness in the implemented recommendations on the bottom-line.
If something does not improve, then we were not successful.
PROACT® includes
a charting
package that will allow the user to
develop their own criteria of measuring performance.
The PROACT® approach is simple, logical and efficient. It
is not full of bells and whistles you will not use. The purpose
of PROACT® is to assist the analyst in developing a "solid
case" built on facts not assumptions. It gets directly to
the point and quick!
Analysis-At-A-Glance is a new feature which provides management
the business information associated with
conducting a Root Cause
Analysis. In this section management
can observe the current
status of any given analysis:
- The % of Data Assigned versus Collected
- The % of Hypotheses
that have been Verified with Hard Data to
be TRUE or NOT TRUE
- The Number of Root Causes
Identified
- The Number of Recommendations Approved
- The Status of Approved
Recommendations
- The Estimated ROI of the analysis
as a whole and for each recommendation.

The PROACT® Suite
Integration Advantage
The valuable outcome of analyses such as FMEA, Opportunity Analysis
and RCA is the knowledge-sharing component. Sure we are all content
when a successful analysis has been completed and the objectives
accomplished. However, are we optimizing our efforts if such
information is not disseminated to those in need in a simple
and practical manner?
LEAP™ and PROACT® RCA were designed to model our respective
methodologies. Therefore, they were designed to work in unison
with each other. The Significant Few events identified in the
LEAP™ program can automatically be bridged into the PROACT® RCA
program. This means that once you have identified
a qualified candidate for RCA you can get down to business immediately
with the RCA.
Within each program, LEAP™ and PROACT® RCA, security
features will allow networked versions to protect "analyses
in progress" from unauthorized viewers. When analyses are
deemed to have been successfully completed and authorized for
distribution, the Principal Analyst of the analysis can "publish" it
so that any other licensed user of the program
can view their completed analysis in a read-only format. Should
the viewer decide they could use some of the information in a
published analysis, they have the option of copying it and renaming
it to suit their needs. At this point they can make any changes
they would like while capitalizing on the knowledge and experience
of others in similar situations.
